  • 1. Ankle sprain
    An actual tear of the ligaments that support the ankle, giving rise to recurrent instability — and hence, more potential sprains — and pain.

    Treatment

    Usually managed non-surgically with RICE (rest, ice, compression, elevation) therapy, ankle braces, physiotherapy and anti-inflammatory medication.
  • 2. Osteochondral lesion
    This is a fairly common and more problematic condition that results from an ankle sprain: damage to the ankle joint surface or talus (one of the two bones comprising the ankle joint).

    Treatment

    The treatment may be surgical or nonsurgical, depending on the nature of the osteochondral lesion.
  • 3. Achilles Tendinitis
    It is the inflammation of the tendon. Rupture of the tendon is also common and can be career ending for athletes.

    Treatment

    Medication, physiotherapy and, sometimes, ultrasound therapy. When it comes to a tendon rupture, the management and repair of it can be non-surgical, depending on its severity. Serious cases may require surgery.
  • 4. Stress fractures
    These are less common ankle injuries. Symptoms are pain, swelling, bruising and problems with ankle motion.

    Treatment

    If you have a stress fracture, seek professional help from an orthopaedic specialist. If the ankle is stable and alignment is normal, surgery is not needed.

Dr Sean Ng is a respected orthopaedic surgeon with experience in the diagnosis and treatment of ankle injury/pain.

Dr Ng completed his orthopaedic foot and ankle training (Ministry of Health Human Manpower Development Program) at the largest and most prestigious hospital in Switzerland, the Hopitaux Universitaires de Geneve (HUG) in Geneva.

Besides being trained in general orthopaedic and trauma surgery, Dr Ng has an active interest in foot and ankle surgery. Other than the management and correction of deformities of common foot and ankle conditions, he is especially interested in percutaneous and minimally invasive surgery of the foot. The aims of percutaneous surgery is to achieve as good a result as open surgery, with minimal scarring, and hence more cosmetically acceptable to the patient.

Dr Ng is the only surgeon able to offer such a service locally. As such, he is also a member of the GRECMIP (European Group for the Study and Research of Minimally Invasive Surgery of the Foot and Ankle).

  • What role do the ankles play in our daily lives?
    Ankles play an extremely important role in the balance and stability of the body and lower limbs. While the feet are wonders of mechanical complexity and strength, the ankles make possible the up-and-down and side-to-side movements of the feet, and act as shock absorbers and propulsion engines for every movement we make with our feet. The ligaments surrounding the ankle joints connect the bones of the legs to each other and to those in the feet.
  • What is a sprain, and what should be done?
    Ankle-related injuries are fairly common, with the ankle sprain – injury to the ligaments – being the most common. The most sensible thing to do in such an instance is to see a doctor, who will establish the nature of the injury with an examination, followed by the appropriate medication and advice.
  • What about fractures?
    Ankle fractures – when one or more of the bones that make up the ankle joint are broken – are also fairly common these days, especially with more people taking up winter sports like skiing and snowboarding. An ankle fracture often has point tenderness over the bone that is fractured, and sometimes results in a very obvious deformity in which the broken parts of the bone physically separate and can be seen. Patients will also often remember hearing a “crack” sound.
  • How can we tell the differences between a sprain and a fracture?
    Sprain: Tenderness and swelling over the outside of the ankle at the soft spot just below the bone.

    Fracture: Tenderness over the outside and inside of the ankle; there may also be deformity of the ankle.

    Sprain: Patient is usually able to walk.

    Fracture: Patient is unable to bear weight and walk properly on the ankle.
